Clinical Education
1 This form is used by undergraduate HHP students in CSDI to document supervised experiences in the practice of speech-language pathology. MSLP students will document hours using CALIPSO.
2 Undergraduate HHP students in CSDI will receive a summary of earned hours in direct service delivery. MSLP students will have access to these summaries in CALIPSO.
3 This form is provided as proof of completion of the required 25 Guided Clinical Observation Hours (CFCC Std. V-C). The Clinical Education Coordinator will verify each student in the MSLP has completed the required Guided Clinical Observation Hours before assigning clients for service delivery. The form will be uploaded to Perceptive, a secure cloud-based platform, and become part of the student’s record.
4 This form is a generic template for Recording Observation Data. Instructors teaching in the HHP CSDI courses may customize written observation forms or may use this generic template.
5 The MSLP program has not yet adopted an EMR system for clinical education and use at the APSU Speech-Language and Swallowing Community Clinic. The Clinical Education Notebook, a physical notebook, is the current method used by clinical educators to ensure undergraduate HHP students in CSDI learn the process and procedures and practice clinical documentation.
