Educational Opportunity Center Intake Application
The Educational Opportunity Center (EOC) program helps adults 19 years old and older, who would like to enroll in college for the first time or who would like to re-enter college after dropping out, by assisting with the college admission process and finding financial assistance for their education. EOC counselors travel regularly and can meet with prospective students at one of our outreach centers. All EOC services are free.
To qualify for our services, a participant must be a U.S. Citizen or permanent resident, reside in a target area (Houston, Montgomery or Stewart County in Tennessee or Christian County in Kentucky) and meet one or more of the following requirements:
- First generation college student (parents do not have a college degree)
- Eligible for a Pell Grant
- Enroll in college for the first time or re-enter after dropping out or stopping out
If you meet one or more of the requirements, complete the following steps:
- Intake Application (through the link provided below)
- Income tax returns (Form 1040, 1040-SR, or 1040-X) for you, your spouse (if you are married), and your parents (if you are a dependent student)
- IRS Verification of Non-tax Filing Letters (if you, your spouse, or parents did not file an income tax return)
- Mail, e-mail, or bring the application and tax documents to our office

Intake Application: Click on the link to generate the application, complete all applicable information and then click on the submit button to transmit the application. You may e-mail your tax documents to eoc-applications@apsu.edu, however, in an effort to protect your Personally Identifiable Information (PII) from inappropriate access, please conceal all birthdates and social security numbers.
If you do not have access to a scanner, you may use your phone to take a picture of your documents and e-mail them to our office. Please be advised, the pictures must be taken in a well-lit environment, they must capture the entire page of each document, and they must be legible.
Appointments:
After an EOC staff member has reviewed your Intake Application and tax documents, you will be contacted to schedule an appointment or be asked to provide additional program eligibility information.
If you are interested in scheduling a virtual appointment on Zoom, you must have access to a desktop or laptop computer with audio and video capabilities. Due to the limited functionality of mobile devices, we are unable to virtually assist participants with completing or reviewing their FAFSA (Free Application for Federal Student Aid).
The Educational Opportunity Center project is 100% funded by the U.S. Department of Education and hosted by Austin Peay State University. I understand that all information provided will be held in strict confidence in keeping with the Family Educational Rights of Privacy Act (FERPA) and is required in order to receive services.
