APSU Veterans Education Benefits Forms
Certification Request Form (CRF)
Required every term. Completed through OneStop — do not use a paper form.
Retroactive VA Certification Request
Use this form if you did not submit a CRF for a prior term, or if the VA backdated your benefit approval (common for Chapter 35). Completed through DynamicForms — log in with your OneStop username and password.
Financial Resources Acknowledgment
Required each term for students on active duty and those using the Post-9/11 GI Bill®, VR&E, or Montgomery GI Bill® programs. This form tells our office what other federal funding (excluding Title IV financial aid) you expect to receive so we can report your tuition and fees accurately to the VA. Submit through DynamicForms if you are unable to complete it through the Online Certification Request.
Tutorial Assistance (TAF)
Available to eligible VA benefit recipients who need tutoring support. The instructor of the course must sign and date the form. VR&E students must receive counselor approval before submitting. Review the Tutoring Program overview (PDF) before completing. Program Overview needs review and an update.
Student Course Requirement Change Form
(For academic advisors and faculty) Completed by an academic advisor or Graduate Coordinator when a student has course deficiencies or prerequisites not listed in their initial Degree Works audit or declared Bulletin.
