6. Reporting Process
Aircraft accidents will be reported in accordance with the FAA and NTSB regulations (Title 49 CFR Part 830). The Aviation Safety Coordinator is responsible all relevant comments are captured and recorded. Any recorded information will be shared with the primary APSU Aviation Safety Committee. All accident data, findings, and recommendation will be released to the organization when authorized.
Personnel who report are treated fairly and justly, without punitive action from the university, except in those cases of “unacceptable behavior (Section 2.7)” or reckless disregard for regulations and standards are involved.
The Aviation Safety Coordinator or designated representative will investigate any occurrence recommended by the Dean of CoSTEM or Program Director/Chief Flight Instructor. Any finding will be shared with the APSU Aviation Safety Committee, prior to sharing the information with the organization.
6.1 – Hazard Reporting Process
The following represents a typical flow to the hazard reporting process and may have to be adapted due to circumstances.

Complete the Aviation Hazard Reporting Form, located in the Appendix of this manual.

The Aviation Safety Coordinator or designated representative shall determine the initial risk assessment value of the hazard. This will be completed after a discussion with the primary stake holder or holders (maintenance, flight, operations).

The Aviation Safety Coordinator shall determine if the hazard/incident should be simply tracked for trends or immediately begin the Safety Risk Management process.

The hazard will receive a control number (yyyydd – 2-digit sequence #, i.e. 202502-01).

The APSU Aviation Safety Committee will be notified of the hazard and its initial disposition.

The Dean of CoSTEM or the Program Director/Chief Flight Instructor will determine if the hazard necessitates shutting down all or a particular operation.

If the hazard is tracked, it may be removed if no other associated reports are received after 12 consecutive calendar months.

If the hazard enters the SRM process, the Aviation Safety Coordinator will assemble the representatives (a minimum of three) to complete the process.

Hazards will be reviewed during the Safety Council Meeting.
6.2 – Accident/Incident Reporting Process
Ground Accident/Incident Reporting Process

If there is a ground incident or aircraft incident, without engine operation and/or rotors turning, complete the Ground Accident/Incident Form located in the Appendix of this manual.

Submit the report to the Aviation Safety Coordinator

Aviation Safety Coordinator and applicable stakeholders will investigate the incident and
-
Determine if a regulatory report is required
-
Gather any applicable information (maintenance, training, weight and balance)

Interview any witnesses
-
Determine any causal or human factors
-
Make any applicable recommendation(s)

Submit the investigation and findings to the Dean of CoSTEM for approval
-
Once approved, share the report with all members of the Aviation Science team
-
Begin implementing corrective actions and track for any new hazards
