Graduate Assistant Forms

- Graduate Assistantship Application
Prospective Graduate Assistants may apply using this form or by submitting an online application.
- GTA Self EvaluationThis form is to be completed every semester by Graduate Teaching Assistants (GTA's).
- Recommendation Form
Students who are applying for a Graduate Assistant position must have this recommendation completed with the application.
- Re-Application for Current Position
A graduate student is eligible to hold an assistantship for up to six (6) semesters and must reapply for each new academic year. Approval from the hiring manager or graduate coordinator is required. Please submit the completed form, to include the approving signature at the bottom, to the College of Graduate Studies. This form is for current graduate assistants only. Current graduate assistants may also reapply using the online application.
- Form for Scholarship Awards in Addition to GA Scholarship
In some cases, graduate assistants are awarded scholarships in addition to their assistantships. If you receive an APSU scholarship that requires additional work hours, please submit this form to the Dean of the College of Graduate Studies upon acceptance of the award.
- Graduate Assistant Request to Add/Reduce Course Hours
The minimum number of credit hours which may be taken in a semester by a graduate assistant is six (6) and the maximum is ten (10). Permission from the Dean of the College of Graduate Studies is required if the credit load is fewer than six (6) or more than ten (10) hours. Please complete this request PRIOR to the beginning of the appropriate semester. Failure to submit the request may result in outstanding fees on your student account.
- Graduate Assistant Summer Semester Out of State Fee Waiver
Graduate Assistants who are classified as out-of-state residents are eligible for summer fee waivers offering in-state rates for summer courses. Submit the request form to the College of Graduate Studies for approval and processing.
- Request for Additional Employment
Students appointed to graduate assistantships may be permitted additional on-campus employment under special circumstances. Additional employment may not exceed 9 hours per week. Final permission is granted by the Dean of the College of Graduate Studies and is not guaranteed. Graduate assistants who are international students may not work additional hours on campus.

This form is provided as an example of how to keep track of the hours worked for personal use. Please check with your department supervisor to verify the method used for tracking the hours you work.
