To change your name with APSU, please complete the PDF version of this Application Update Form. This form must be submitted with a photo ID and an official government document-ie. Social Security card, marriage certificate, or other court order.  These documents can be mailed to: Austin Peay State University, Office of Admissions, Box 4548, Clarksville, TN 37044.
