Certificate
of Understanding (COU):
This form lists DVA and APSU guidelines regarding
your VA Educational
Benefits. This form must be
submitted with your application for VA Educational
Benefits.

Certificate of Understanding
(COU) for VA Vocational Rehabilitation:
This form lists VA Vocational Rehabilitation and
APSU guidelines regarding your VA Vocational
Rehabilitation educational benefits. This form must
be submitted to our office during your first term of
enrollment.

Certification
Request Form (CRF): This form is required
EVERY term
of enrollment, including the FIRST term
of enrollment. This
form indicates to the OVA which classes you plan
to pursue/are pursuing for a specific term of
enrollment. This form indicates your desire
for certification for that specific term.

Change
in Status (CIS): This
form must be completed whenever there is a change
in your enrollment status, i.e., drop/add course
or withdraw from the University. If you did not
list a course on your original CRF and you would
like to request certification, OVA will not be
able to certify this course without you completing
this form. If you have to
withdrawal due to mitigating circumstances, you must
claim it on the CIS and attach supporting
documentation.

Advance Payment Agreement
Form:
Advance payment is the VA
educational assistance allowance paid to the
student for the first partial (at a prorated
amount) month and the following full month
of enrollment. Advance payment
can only be requested by students initially enrolling or
students who have been absent from school
for a semester/term that
is more than 30 calendar days and did not
receive break pay.
If you request Advance Payment, our
office will not be able to process your request
without this form. For

Statement of Understanding

Tutorial Assistance
Internal Recommendation Form (TAF):
Please read over the
VA Tutorial Assistance brochure and speak to
your appropriate VA counselor at main campus or Ft
Campbell Center.
This
form is required for students needing tutorial
assistance. This form must be signed and dated by
the instructor of the course the student requests
tutoring for and is submitted to the OVA, in order
to validate the need for tutoring.
VA Vocational Rehabilitation students
must
get approval from their counselor.

OVA Deferment Form:
This form is only available in person. VA
eligible beneficiary may elect, upon formal
application, to defer payment of required tuition
and fees only. However, the deferment cannot
exceed the total monetary benefits to be received
for the term. Housing, books, supplies and meal plans are
not deferred.
