Absence Form

This is to be filled out and signed by the GPC President in order to be excused from a GPC meeting. If an absence is not excused in this manner, it may count against you.

This form, taken from the Governor's Guide, is designed to help you take into account all of the considerations when planning a program.

GPC Program Proposal

This form must be filled out and turned in to the Secretary/Treasurer in order to propose your program at a GPC meeting.

• Chair of committee fills out proposal
• Be sure to fill in all the logistics of the event
• Once paperwork is completed, it must get the appropriate signatures.

GPC Program Evaluation

This form should be filled out after an event, to evaluate the strengths and weaknesses of a program to see how it can be improved in the future.
