As a part of the sign up process for each trip, each participant must complete the following form:

Before beginning to climb or boulder, all participants must complete the following two forms. Anyone under age 18 must have the signature of a parent/guardian.

To play in an intramural league or tournament, please sign up using the form that corresponds to the event (team or individual/dual).

If you want to play in a team event, but do not have a team, you can submit a Free Agent Form. Free agents may be picked up by teams or grouped together for an independent team

If the event/activity takes place off campus, travel forms will also need to be completed, as they apply.

In order for University affiliates to access University Recreation facilities and services, the following forms must be completed:

        University employees receiving full benefits)
