Instructions: There are two "evaluation forms" required for the internship. They are shown on this page, and available as a cleaner PDF file here. If you use those displayed below instead of the pdf, it is suggested you "File-Save as" this page or Copy-and-Paste into a word processor and work on it until you get the formatting suitable for printing, eliminating extraneous information, just leaving the forms. Then, after filling out the information that you, the student, should fill out, give both the midterm and final evaluation forms to your site supervisor. Then, have them delivered to the instructor, or if the site supervisor prefers, give them the mailing address or fax number of the instructor. Please thank them on behalf of the instructor and the university.
