You will be considered for funding from NSF if you do the following:

                                         1. Complete & submit this application form.
                                         2.  Attach a resume to an email sent to MaPS@apsu.edu.
                                         3. Complete your FAFSA.  http://www.fafsa.ed.gov
                                         4. Declare Mathematics and/or Physics as your major at APSU  .
                                         5. Request two individuals who are familiar with your academic abilities submit recommendation forms.
