Forms for Dual Enrollment Applicants
We will begin accepting online forms for Summer 2019.
If you are applying for Spring 2019, you can find our paper forms here.
Please submit one high school recommendation form for each new dual enrollment student. Send transcripts to APSU through Parchment, SendEDU, or directly to email@example.com. You do not have to submit this form for returning students.High School Recommendation Form
If your student is new to dual enrollment with APSU, please complete the Parent Acknowledgement Form.Parent Acknowledgement Form
If you are continuing dual enrollment after your first semester with APSU, please complete the Returning Student Form. You'll need your parent present to sign electronically, too.Returning Students Form
Once you send in your required forms, you'll need to register for classes through OneStop.
