Financial Aid - Forms

  • Additional Financial Aid Form - This form is used to notify the Financial Aid Office of any additional expected financial aid resources that are not indicated on your award letter.

  • Adjustment Form for Loans - This form is to be completed by students requesting the additional unsubsidized stafford loan after PLUS has been denied, students that want to reduce or increase their loan amounts (previously accepted award online), or students who want to cancel their loan (previously accepted). If a student failed to choose a lender when accepting their award online, students can indicate their lender on this form.

  • Authorization Form - Authorizes the use of federal financial aid to pay prior term charges within the same academic year.

  • Change of Campus Enrollment - This form is to be used when you are requesting to change your award to reflect a different enrollment period. This request will require necessary changes to your budget and could possibly change the amount and number of scheduled disbursements for your financial aid award.

  • Dependency Override Form - This form is to request an override of a student's Dependency Status determined by the US Department of Education. (Supporting documentation is required.)

  • Dependent Institutional Verification Worksheet (IVF-D) - This worksheet is used to verify the information reported on your FAFSA with copies of your and your parent(s) federal tax forms, W-2's or other financial documentation.

  • Eligible Borrower Form - If you had loans discharged due to disability and are reapplying for loans, this form must be completed by your physician.

  • Fee Discounts for Dependents of Licensed Public School Teachers and Dependents of State of Tennessee Employees - This form must be submitted to the Business Office in the Browning Building.

  • Fee Discounts for Employees of the State of Tennessee - This form must be submitted to the Business Office in the Browning Building.

  • FWS Request Card

  • GRAD PLUS loan request sheet - This form is to request a graduate plus loan for graduate students.

  • Independent Institutional Verification Worksheet (IVF-I) - This worksheet is used to verify the information reported on your FAFSA with copies of your (and your spouse's, if married) federal tax forms, W-2's or other financial documentation.

  • Lottery Scholarship Appeal Form - The HOPE Lottery Scholarship is awarded based on policies set forth by the Tennessee Student Assistance Corporation (TSAC). TSAC's HOPE Lottery Scholarship policy allows an appeal process for students who fail to meet initial or continuing eligibility.

  • Lottery Scholarship Course Repeat/Exclusion Consent Form - The HOPE Lottery Scholarship rules allows for a student to raise their GPA by repeating a class and having the original grade excluded from your HOPE Lottery Scholarship GPA. This is only for ONE time.

  • Low Income Document - This form is used to verify the information reported on your FAFSA.

  • PLUS loan - The PLUS loan is a low variable interest rate loan made available to parents of a dependent undergraduate student.  Parents will generally be required to pass a credit check when applying for the PLUS loan.

  • Request for Corrections of Data Form - Only complete when correction needs to be made to a Student Aid Report.

  • Satisfactory Progress Appeal (SatProg) - This form must be completed when a student has been notified by the Student Financial Aid Office that they are in violation of the Satisfactory Policy Guidelines.

  • Satisfactory Progress Policy Guidelines - Federal Guidelines which must be followed by, in order to receive federal aid and/or loans.

  • Scholarship Applications:

  • SMART Advisor Form - This form will required of SMART grant recipients. The student's Academic Advisor will complete the form and the student will submit it to the Financial Aid Office.

  • Special Considerations Form - This form is to be completed only if a significant change in your family circumstances occurs. For example, loss of parent/student/spouse's income, separation or divorce of student/parents, death of parent/spouse. (Supporting documentation is required.)

  • Summer Financial Aid - Application for Summer 2010 is available.

  • TN Student Assistance Corp. Change of College Request - This form is used to notify the TN Student Assistance Corporation (TSAC) that you want to transfer potential eligibility for state funded grants, scholarships, and loan forgiveness programs from another school to APSU. The form should be faxed or mailed to the address on the form.
