Financial Aid - Forms

  1. Authorization Form - Authorization for APSU to credit all allowable Financial Aid to a student's account.
  2. FWS Request Card

  3. Satisfactory Progress Policy Guidelines - Federal Guidelines which must be followed by, in order to receive federal aid and/or loans.
       
  4. Satisfactory Progress Appeal (SatProg) - This form must be completed when a student has been notified by the Student Financial Aid Office that they are in violation of the Satisfactory Policy Guidelines. 
      
  5. Summer Financial Aid Application - Application for Summer 2008 will be available March 2008. 

  1. Dependent Institutional Verification Worksheet (IVF-D) - This worksheet is used to verify the information reported on your FAFSA with copies of your and your parent(s) federal tax forms, W-2's or other financial documentation. 

  2. Independent Institutional Verification Worksheet (IVF-I) - This worksheet is used to verify the information reported on your FAFSA with copies of your (and your spouse's, if married) federal tax forms, W-2's or other financial documentation.  

  3. Scholarship Applications:
    • Academic Scholarship Application - Available for first-time freshmen through the APSU Admissions Application.
    • Community College Scholarship Application - Available for Tennessee Community College and Hopkinsville Community College students.
    • General Scholarship Application - Applications will be accepted from September 1st through February 1st of each year.

  5. Lottery Scholarship Appeal Form  The Tennessee Education Lottery Scholarship (TELS) is awarded based on policies set forth by the Tennessee Student Assistance Corporation (TSAC).  TSAC’s TELS policy allows an appeal process for students who fail to meet initial or continuing eligibility.

  6. Lottery Scholarship Course Repeat/Exclusion Consent Form The TELS HOPE rules allows for a student to raise their GPA by repeating a class and having the original grade excluded from your TELS/HOPE GPA. This is only for ONE time.    

  7. PLUS Loan - Parent Loan application for dependent students. 

  8. GRAD PLUS loan request sheet - This form is to request a graduate plus loan for graduate students.
         
  9. Adjustment Form for Loans - This form is to be completed by students requesting the additional unsubsidized stafford loan after PLUS has been denied, students that want to reduce or increase their loan amounts (previously accepted award online), or  students who want to cancel their loan (previously accepted). If a student failed to choose a lender when accepting their award online, students can indicate their lender on this form. 

  10. Change of Campus Enrollment - This form is to be used when you are requesting to change your award to reflect a different enrollment period. This request will require necessary changes to your budget and could possibly change the amount and number of scheduled disbursements for your financial aid award.

  11. Request for Corrections of Data Form - Only complete when correction needs to be made to a Student Aid Report. 
       
  12. Special Considerations Form - This form is to be completed only if a significant change in your family circumstances occurs.  For example, loss of parent/student/spouse's income, separation or divorce of student/parents, death of parent/spouse.  (Supporting documentation is required.)
      
  13. Dependency Override Form - This form is to request an override of a student's Dependency Status determined by the US Department of Education.  (Supporting documentation is required.)
  14. Additional Financial Aid Form - This form is used to notify the Financial Aid Office of any additional expected financial aid resources that are not indicated on your award letter.

  15. Fee Discounts for Employees of the State of Tennessee - This form must be submitted to the Business Office in the Browning Building.

  16. Fee Discounts for Dependents of Licensed Public School Teachers and Dependents of State of Tennessee Employees - This form must be submitted to the Business Office in the Browning Building.

  17. Eligible Borrower Form - If you had loans discharged due to disability and are reapplying for loans, this form must be completed by your physician.
