US $65 transportation fee if transportation is needed from Nashville airport to APSU
For transfer students from another US school, college or university, include a photocopy of I-20 form, I-94 form, US Visa page and passport information page, release form from current school
Official high school or college transcript in English

A certificate from a licensed physician or
other qualified medical authority certifying that you are free from
tuberculosis
Proof of two Measles, Mumps and Rubella (MMR)

I-20 and VISA: An I-20 form (Certificates of Eligibility for F-1 visa) will be issued to international students who have been accepted by the ESL Institute. It is the responsibility of the student to use the I-20 form to apply for the F-1 student visa from the nearest U.S. Embassy.
A fee of $60 will be assessed to cover
express mail charges for the I-20 document and will be payable with
tuition and fees at the time of registration.
Part-time students should submit a completed part-time
application form and the US $106.25 per unit hour tuition fee to the address
above.
