Distance Education Course Development Forms and
Contracts
This page lists the links you will need to submit a
proposal for a course delivered in a distance setting and the
associated contract information. *All
Department Chairs are responsible for submitting all documents
necessary for online course development to Dr. Groppel, Dean, Center
for Extended and Distance Education. Course development must be
approved prior to beginning work on the course.
You will need to submit the forms in this order:
1. Distance
Education Course Proposal Sheet, (PDF), (Word) print this form, complete it
and forward the completed proposal with the appropriate contract. You may review the
contracts at:

(PDF) (Word) you may select the most appropriate of the five options. This
form, too, must be printed in order to be completed.

print this form, complete it and forward along with other forms.
The proposal sheet and contract will be reviewed by the Dean of Extended and Distance
Education and the Instructional Technologist.
4.
Evaluation of Course
by Course Developer (Self Evaluation), please fill out and
submit this form to the Dean of Extended and Distance Education and
the Instructional Technologist, after the course is developed and
before the end of the semester in which it will be taught.
5.
Evaluation
of Course by Peer (Peer Review Evaluation), please have a
peer chosen by the department fill out and submit this form to the
Dean of Extended and Distance Education and the Instructional
Technologist, after the course is developed and before the end
of the semester in which it will be taught.
6.
Evaluation of Course by Chair
(Chair Evaluation), please have your Department Chair
fill out and submit this form to the Dean of Extended and Distance
Education and the Instructional Technologist, after the course is
developed and before the end of the semester in which it will
be taught.

Please use the following as a guide while developing your course.
*All documents must be submitted to
Dr. Groppel, Dean, Center for Extended and Distance Education prior to
beginning course development. Payment for course development is
made at the end of the first term in which the course is taught.
As part of our efforts to make the development of distance education
courses reflect the mission of the university an Online Course Review Team represented by
faculty, staff and students is active in defining and refining course
development and course offerings. While concentrating on online
courses at this time, the efforts of this team will include decisions to be considered as models
for all courses offered in a distance environment. Your input is
welcome, please contact the Distance Education office for more
