To complete your Credentials File, you must submit the following:

1. an electronic copy of your APPROVED resume neatly presented in Word or PDF format*

2. an electronic copy of three to five references neatly presented in Word or PDF format*

3. a Personal Data Sheet in hardcopy or electronic format.

4. an Authorization Form in hardcopy or electronic format.  This form requires your signature, so, if you wish to submit it electronically, please print, sign and scan it.

*The job market of the twenty-first century is online.  Therefore, we now maintain electronic credentials files.  You must submit a digital  copy of your resume and reference list.  If you need assistance preparing these materials, please contact our office.
