Completing Your Credentials File

To complete your Credentials File, you must submit the following:

• an electronic copy of your APPROVED resume neatly presented in Word or PDF format*
• an electronic copy of three to five references neatly presented in Word or PDF format*
• a Personal Data Sheet in hardcopy or electronic format.
• an Authorization Form in hardcopy or electronic format. This form requires your signature, so, if you wish to submit it electronically, please print, sign and scan it.

*The job market of the twenty-first century is online. Therefore, we now maintain electronic credentials files. You must submit a digital copy of your resume and reference list. If you need assistance preparing these materials, please contact our office.

Please complete all items thoroughly.

INCOMPLETE FILES WILL NOT BE ACCEPTED.
